Job Description

Saint Joseph's Medical Center is seeking a full time Physical Therapist Assistant (part-time if preferred) competitive rates for our School Based Program for the school calendar 2023-2024.
Must be willing and able to travel between schools and homes.
The School-Based Rehab Program at Saint Joseph's Medical Center provides physical and occupational therapy services for children and adolescents 5-21 years of age with a variety of diagnoses. Our unique program offers services in a stimulating environment for a diverse population while collaborating with educational staff and professionals in the Yonkers Public Schools. It is conveniently located close to New York City and most major highways and transportation. There are over 50 highly skilled PTs, OTs, COTAs, and PTAs on our team.

Position description: Intervention for children in the schools and/or in their homes; Documentation of sessions and progress; Preparation of Individualized Education Plans (IEPs); Maintenance of ongoing communication with parents, guardians, and teachers all under the supervision of a certified physical therapist.

Candidates should have strong interpersonal skills, the ability to work independently, and strong written and verbal communication skills. They must have excellent organizational skills and be flexible.

POSITION DESCRIPTION:
The Licensed Mental Health Clinician is responsible for providing individual and group counseling services on a wide range of clinical issues for individuals diagnosed with an Opioid Use Disorder. The Licensed Clinician assists patients with accessing community resources and services related to their diagnosis such as legal, medical, and vocational/educational needs.
POSITION FUNCTIONS:
· Complete and/or review Intake Assessments.
· Conduct individual assessments and counseling to assigned caseload addressing family, medical & psychiatric concerns.
· Conduct evidence-based group counseling services related to a wide range of clinical and community issues.
· Maintain up-to-date charts including assessments, care plans, case notes and documentation of collateral contacts.
· Provide crisis intervention services.

The Clinical Supervisor ensures the quality of treatment services, the fidelity of evidence-based service delivery and timely documentation of all group and individual counseling services and support services delivered by the facility’s clinical staff. The Clinical Supervisor promotes an atmosphere that encourages individualized services and consumer involvement in therapeutic activities.
