Job Summary:

Under general supervision, performs the full scope of duties associated with professional pharmacy practice. Is empowered to use judgment in the decision-making process within their sphere of activity.

Requirements:

Graduation from an accredited School of Pharmacy (BS or Doctor of Pharmacy) plus current license and registration to practice in New York State.

PharmD or MS degree and/or completion of Pharmacy Residency Program may be used as a substitute for some experience.

Minimum of three years' experience as a pharmacist in Hospital Pharmacy with accepted standards of practice, outpatient experience is preferred.

Maintains professional competence through participation in continuing education programs and review of periodical literature.

Maintains and provides proof of 15 Continuing Education credits each year.

Description
POSITION SUMMARY: Under the direction and supervision of the CCBHC Medical Director, the NP will provide medication assisted treatment (MAT) consultations and assess for the need of Buprenorphine (Suboxone). The NP will provide education and information regarding effects, dosing, and treatment, and when applicable, induction and treatment follow-up. The provider will be based in an outpatient clinic with services being offered telehealth or on-site at shelters, harm reduction sites, and outreach programs.





POSITION DESCRIPTION:



§ Provide MAT consultations, education, and treatment to address opioid use disorders in accordance with statutes, regulations, and protocols of the profession, OASAS, and CCBHC



§ Deliver high-quality, patient-centered care in a trauma focused approach



§ Design/participate in the development of individualized treatment plans, incorporating best practice into treatment interventions



§ Ability to maintain strict patient confidentiality in compliance with HIPAA regulations



§ Refer patients based on clinical, medical, or other needs and provide updates to applicable referents as needed



§ Ability to lead, manage, and support a multidisciplinary team effectively



· Document and bill all interaction recorded in the electronic health records (EHR)



· Attend all agency staff meetings and annual in-service as required or mandated.



· Maintain good working relationships with all agency staff, patients and visitors.

Physical examinations with appropriate referrals to Health Services and/or specialists.
Modify prescribed methadone dosage take home schedule with recommendations to Medical Doctor.
Follow-up evaluation at weekly intervals for the first two weeks after admission to assess how well the client has adjusted to dosage and whether there is any evidence of drug idiosyncrasies or patient abuse.
Medical documentation which includes but is not limited to the following:
Medical and mental health histories
Medication: current and past
Physical findings
Medical and mental health diagnoses
Referrals to specialists
Laboratory orders and findings
Electrocardiograms
Diagnostic procedures including imaging studies
Surgical histories

The SUV Stand Up to Violence is a gun violence prevention initiative run out of NYC Health + Hospitals/Jacobi. The SUV program is an evidence-based, violence reduction initiative that treats gun violence as a disease, by identifying its cause and it's causes and interrupting its transmission. The program hires a community outreach team of credible messengers - individuals who are hired from the communities in which they work and have backgrounds similar to those with whom they aim to connect - mentor the highest risk youth with an emphasis on conflict mediation and violence prevention.




In order to prevent violence, methods and strategies associated with disease control are implemented:


1. Detecting and interrupting conflicts - Outreach Workers (OW) identify and mediate conflict between high-risk individuals and groups in order to prevent violence and ongoing acts of retaliation.

2. Treating the highest-risk individuals the OWs mentor and support high-risk youth and young adults (ages 14 to 28) and connect them to educational, vocational, recreational and social services.

3. Changing social norms.


Opportunity Details:

Recruit and manage a caseload of 15 "high risk" gang/crew members and provide case management services. Responsible for mentoring these participants with a focus on providing alternatives to high-risk and violent behavior

Build and maintain relationships with local gangs/crews, stay up-to-date on rivalries and plans for retaliation, and facilitate conflict mediation.
Canvass the target areas on foot and identify and diffuse "hot spots" for shootings and violence.
Link participants and individuals in the target area to opportunities and programs in the community including vocational, educational, recreational, and social services.
Plan and participate in monthly community activities (cook-outs, basketball tournaments, etc.) in order to strengthen community awareness and garner support.
Engage in community shooting responses (marches, vigils, etc.) within 72-hours after a shooting, in order to mobilize the community against gun violence and change community norms.
Document all participant interactions and conflict mediations in the program's computer database.
Attend ongoing trainings, individual supervision, and weekly staff meetings.
Communicate effectively with the team including other OWs and supervisors.
Adhere to program safety regulations and remain in contact with a supervisor at all times.
Work flexible hours (mostly evenings and weekends) and respond to shootings or emergency situations.
Respond to violent trauma incidents and see patients in the hospital, when needed.
Qualifications
Familiar with gang/crew members and specific high-risk individuals and seen as "credible" in the communities where we work.
