The Counselor position provides individual and group counseling and case management to individuals receiving integrated substance abuse treatment and primary care services at the Montefiore Medical Center.
Responsibilities
Develop a comprehensive psychosocial and complete upon admission.
Instruct patients in program policies and procedures and document in the clinical chart.
Develop, implement and review of treatment plans.
Provide substance abuse counseling to individuals, groups, and families focusing on building motivation, educating, planning crisis intervention, relapse prevention and aftercare.
Care and case management of comprehensive services for patients.
Monitor patients' response to methadone medication treatment through individual counseling and toxicology review.
Conduct vocational/educational and learning needs evaluation in psychosocial and ongoing as indicated. Provide referral as needed.
Perform preliminary screening regarding mental health needs of patients on caseload (i.e., presence of psychiatric symptoms, suicidality, homicidality, etc.).
Evaluate patient eligibility for entitlements (Public Assistance, SSI, Medicaid etc.).
Refer patients to appropriate agencies when indicated.
Enter patient data into HRA/STARS computerized database in a timely manner.
Act as liaison between patient and outside agencies.
Attend regularly scheduled meetings. i.e., IDC's, staff meetings, morning reports, case management meetings, and other team meetings determined to be necessary.
Maintain required records on patients and updates progress notes as required. Ensure clinical chart contains necessary information regarding patient's status and other information. Complete all required forms and treatment plans.
Assist patients with referral to other services and agencies determined to be necessary to the recovery process (i.e., shelters, detox units, legal counsel, etc.).
Give case presentations in supervision, IDC meetings, daily reports.
Ensure all counselor specific billable services are billed and perform follow-ups to billing (delinquent payments, obtaining information required to bill, confirming, and updating billing information, etc.).
Directly supervise the collection of toxicology samples for all patients when required.
Meet with family members, significant others and outside providers as determined to be necessary and appropriate for clinical management and care.
Requirements
Bachelor's Degree required, Master's Degree Social Work, Mental Health Counseling is preferred.
Candidates with a Bachelor's degree must have a CASAC.
Candidates with a Master's degree must have one of the following NYS Licenses or certifications at all times in order to work:
CASAC
MSW with limited permit
MHC with limited permit
MFT with limited permit
LCAT with limited permit.
A minimum of 1 year of work experience in addiction counseling required.
Knowledge of computer software and ability to document all services in an Electronic Health Record.
12 core functions of Counselors, knowledge of governmental, private, and community programs and entitlements available to patients, familiarity with Oasis and TJC regulations and standards are required.


The regular hours are 8:00am - 4:00pm and must be able to work various shifts and locations depending on operational needs.

Physician Affiliate Group of New York (PAGNY) has an opportunity for an Endovascular Fellow in the division of Cardiology at NYC Health + Hospitals/Jacobi and North Central Bronx. Jacobi Medical Center is a 457-bed, Level I Trauma Center and North Central Bronx Hospital (NCB) has 213 licensed beds. The hospital maintains a strong academic affiliation with Albert Einstein College of Medicine to maintain its high healthcare delivery standards.

The Division of Cardiology has had its own fellowship in Vascular and Endovascular Medicine for over a decade and is also a major teaching affiliate for Montefiore’s Cardiovascular Disease Fellowship Program. Jacobi is a New York State certified STEMI receiving center with an active PCI program and a mechanical support program including ECMO. Jacobi has outstanding cardiovascular clinical facilities offering a full range of diagnostic and therapeutic procedures including coronary and peripheral vascular interventions, pacemaker, ICD and BiV ICD implantation, and cardiac CT / MRI. Jacobi also has had an active Pulmonary Embolism Response Team (PERT) since 2014.

Opportunity Details:

Jacobi Medical Center is offering a 1-year Endovascular fellowship.
Fellowship focuses on arterial and venous disease management using endovascular techniques.
Training also includes:

Percutaneous interventions for acute pulmonary embolism.
Wound management.
Endovenous ablation.
Interventional dialysis access management.
Formal training in vascular ultrasound interpretation.
Training provided by Jacobi’s IAC Vascular Testing certified noninvasive lab.
The Jacobi experience is unique given the close collaboration between interventional cardiology and vascular surgery to provide a very comprehensive didactic curriculum and hands on training with both endovascular cardiologists and surgeons.

The SUV Stand Up to Violence is a gun violence prevention initiative run out of NYC Health + Hospitals/Jacobi. The SUV program is an evidence-based, violence reduction initiative that treats gun violence as a disease, by identifying its cause and it's causes and interrupting its transmission. The program hires a community outreach team of credible messengers - individuals who are hired from the communities in which they work and have backgrounds similar to those with whom they aim to connect - mentor the highest risk youth with an emphasis on conflict mediation and violence prevention.




In order to prevent violence, methods and strategies associated with disease control are implemented:


1. Detecting and interrupting conflicts - Outreach Workers (OW) identify and mediate conflict between high-risk individuals and groups in order to prevent violence and ongoing acts of retaliation.

2. Treating the highest-risk individuals the OWs mentor and support high-risk youth and young adults (ages 14 to 28) and connect them to educational, vocational, recreational and social services.

3. Changing social norms.


Opportunity Details:

Recruit and manage a caseload of 15 "high risk" gang/crew members and provide case management services. Responsible for mentoring these participants with a focus on providing alternatives to high-risk and violent behavior

Build and maintain relationships with local gangs/crews, stay up-to-date on rivalries and plans for retaliation, and facilitate conflict mediation.
Canvass the target areas on foot and identify and diffuse "hot spots" for shootings and violence.
Link participants and individuals in the target area to opportunities and programs in the community including vocational, educational, recreational, and social services.
Plan and participate in monthly community activities (cook-outs, basketball tournaments, etc.) in order to strengthen community awareness and garner support.
Engage in community shooting responses (marches, vigils, etc.) within 72-hours after a shooting, in order to mobilize the community against gun violence and change community norms.
Document all participant interactions and conflict mediations in the program's computer database.
Attend ongoing trainings, individual supervision, and weekly staff meetings.
Communicate effectively with the team including other OWs and supervisors.
Adhere to program safety regulations and remain in contact with a supervisor at all times.
Work flexible hours (mostly evenings and weekends) and respond to shootings or emergency situations.
Respond to violent trauma incidents and see patients in the hospital, when needed.
Qualifications
Familiar with gang/crew members and specific high-risk individuals and seen as "credible" in the communities where we work.
